Discharge Time and Procedures

Discharge Information

Your discharge will be planned by the admitting consultant. Every day you will be briefed about your progress and given an estimated discharge date. Discharge can only be processed on the doctor's written advice.

Procedure

All the admitted patients in AEH will have a treatment plan and a discharge plan. The discharge procedures will begin once primary consultant advices to discharge the patient.

At the time of discharge, our staff will ensure that:

  • Follow up appointments are arranged, if needed.
  • You have received the discharge medications and explained the usage, as well as the side effects of the medications.
  • Basic self-care procedures and health education has been given to you.
  • Every patient will be provided with a summary of their treatment at the time of discharge.
  • Our staff will call the patient after 24hrs of discharge to check the condition of the patient.

Please be reminded of the following:

  • Follow the medication instruction.
  • Ensure that you do not miss the follow-up appointment.
  • Check your bedside cabinet, to ensure that you have not left any items behind.
  • Withdraw any personal items that you may have placed in the safe.
  • Return all the items which belongs to the hospital.

Discharge Time

  • Usually discharge advice is given by the doctor during ward rounds between 8 AM to 10 AM.
  • Preparation of discharge document is between 10 AM to 12 PM.
  • Discharges will be processed between 12PM - 2PM.
