Leaving AEH: A Smooth Transition Home

This guide explains what to expect when you're discharged from AEH. We want to ensure a smooth transition back home for you.

Planning for Your Discharge

Discharge Process:

  • All patients have a treatment and discharge plan. Your discharge begins when your doctor advises it.

  • Our staff will ensure everything is in order before your leave, including:

    • Scheduling follow-up appointments, if needed.

    • Providing discharge medications with detailed instructions on use and potential side effects.

    • Offering basic self-care instructions and health education.

    • Giving you a summary of your treatment.

  • We'll also call you within 24 hours of discharge to check on your well-being.

What to Remember Before Leaving:

  • Take all discharge medications and follow instructions carefully.

  • Don't miss any scheduled follow-up appointments.

  • Double-check your bedside cabinet and safe for personal belongings.

  • Return any borrowed hospital items.

Typical Discharge Time frame:

  • Discharge advice is usually provided during morning rounds (8 AM - 10 AM).

  • Discharge paperwork is prepared between 10 AM - 12 PM.

  • You can expect to be discharged between 12 PM - 2 PM.
